Bio-identical Hormone Replacement Therapy

Hormone replacement therapy (HRT) is most often used to treat menopausal symptoms. Typical symptoms of menopause would include hot flushes, vaginal dryness, low libido, weight gain, irritability, moodiness and sleep disturbances. Women have traditionally used synthetic estrogen to treat menopausal symptoms and to help protect against osteoporosis. A second option now, lies in the use of bio-identical HRT through the use of plant-derived hormones that are biologically identical to mimic hormones naturally occurring in women.

Bio-identical HRT is however, not limited solely to the treatment of menopause and can also be used to treat or alleviate symptoms of endometriosis, Pre-Menstrual Syndrome, weight gain, low libido or fibrocystic breast disease.

A simple look at physiological functioning would reveal that no two women are exactly alike! The value of bio-identical hormone replacement therapy is that it can be adapted to fit individual body and hormone levels. Such hormones can be made in a variety of strengths and dosage forms beyond the commercially available synthetic hormones. Commonly replaced hormones include estradiol, estrone, estriol, progesterone and testosterone.

Men, too, benefit from hormone replacement therapy as part of an anti-aging treatment regime! Both men and women undoubtedly experience symptoms related to reduced production of sex hormones (sex steroids) as they get older. These changes are inevitable, but affect individuals differently.

Some people experience very few symptoms, whereas others are completely disabled by them. It is important to remember that this fall in sex hormone production is a natural process and not a disease. However, many unpleasant natural processes can be helped with medical treatment just as effectively as diseases.

In men, the fall in sex hormone production is much more gradual, developing over decades rather than months or years. Mental and physical changes can occur, but they are much more subtle in onset and can easily be missed.

As such, the term 'male menopause', or andropause, is probably not accurate. Instead, experts prefer to talk about 'partial androgen deficiency of the ageing male' (PADAM).

Production of testosterone (the principal male-determining sex steroid) falls gradually and progressively from the 40s onwards. Other hormones are also affected, including growth hormone, insulin-like growth factor-1 (IGF-1), parathyroid hormone and melanocyte-stimulating hormone.

Formulations of bio-identical hormones include capsules, topical creams and gels, pessaries and sublingual troches or lozenges.

With the help of a doctor and a compounding pharmacist, one can start and maintain a bio-identical hormone replacement regimen that closely mimics what one’s body has been doing naturally for years.

In tailoring the regimen, symptoms are a simple way of gauging the adequacy of the hormone replacement therapy. Blood or salivary hormone testing ensures that the replacement therapy is not excessive and that the body is metabolizing the hormones well, so as to reduce the likelihood of side effects.
